Supervisor Application Instructions

These instructions are for supervisors of NZCCA members or membership applicants who are not yet approved to supervise NZCCA members.

You can find our supervisor's competence and experience criteria on our Supervision Requirements webpage. 

Supervisor Sign up instructions:

(only if you do not have a login to the NZCCA website)

  1. Go to the sign up page on the NZCCA website Add your details as requested.
  2. Go to and enter your email address followed by ‘Send me the password reset link’
  3. Go to your email inbox and click on the email from either or (this may take a few minutes to arrive). Please check your spam or junk folder if you cannot find it.
  4. Click on the password reset link in the email
  5. Enter your new password.
  6. Set up MFA (Multi-Factor Authentication).  Click here for Help.


Completing the Supervisor Application Form:

Once you are logged in go to the Supervisor Application Form (you need to be logged in to view this form).

  • Complete the information as required.
  • Address of Clinical Practice:

Street 1 = your practice/business name (if any)

Street 2 = your street address

Street 3 = Rural delivery number e.g. RD2 (if applicable)

  • Save & Submit the form.
  • The Membership Committee will review your application and approve or decline based on if you meet our supervisor competence and experience criteria.  This usually takes 2 or 3 days.
  • The NZCCA Administrator will email you once your application has reviewed.
  • Please notify your NZCCA supervisees that you are approved so they can select you as their supervisor.
